A good place to start with developing the app is by starting where the patient will star--the pre treatment questionnaire. Now, I don't expect patients to know their entire medical history and we certainly don't want them guessing. For that reason, it would be best for patients to begin by answering a few simple questions. I made a list, based on the info I've come across during web-based research and the experience that I had in Dr. Perumalswami's office and Dr. Weiss's support group. Here are the questions that I came up with, but I expect to add more after hearing back from the team.
I. Patient profile
a. Upload picture
b. Name
c. DOB
d. Stage of HCV (pre/current/post)
II. Questionnaire
a. Genotype: 1/2/3/4
b. Date of Diagnosis: MM/DD/YYYY
c. Prior treatment? Yes/No
d. Response if Yes? Relapse/Partial/None
e. Other medical conditions? search through database of registered conditions or enter other (eg: HIV, diabetes)
f. Stage of Liver Disease: idk, 1/2/3/4
g. Do you smoke? Yes/No
h. Do you drink? yes/no
i. Are you a drug user? yes/no
j. If yes, which? marijuana, cocaine...search through database or enter other
k. are you currently on any other medications? none/select from database
l. Tell us more about yourself and how HCV has affected your life. (eg: I'm a single mother with unstable housing....)

IV. My Doctors
a. The patient can either search the database for his doctor or find one
b. The patient will begin by typing the name of his doctor and a list of registered doctors will appear. The patient will select his doctor and all of the contact info is already in the system.
c. If the doctor cannot be found in the system, the patient will need to enter all info about the doctor. Including, name, address, phone number, and email address.
d. If the patient doesn't have a doctor, he can search for one by entering his city, state or zip code. Google maps will offer registered doctors for the patient to chose from. The patient can view doctor profile and pick one that is best for his needs.
V. Send a request.
a. After the doctor has been chosen and contact info entered, the patient will send a request to the doctor. From the request, the doctor will verify that that's his patient, or he'll admit the patient under his care.
b. The doctor will be sent a list of pretreatment information about the patient, including medical history, etc.
c. Form the request, the PCP will have full access to the patient's account and will upload all medical records to the "about me" portion of the patient's app. The PCP will enter the info via website, the patient will see it via app.
d. The doctor info will be registered into the icon on the app to contact the doctor.
